One: ask reviewers to leave one comment per change, not a list. Threads are cheaper to resolve than essays.

Two: resolve as you go. A comment marked resolved disappears from the timeline marker color, which lets editors see at a glance what is still outstanding.

Three: lock the approved version. Once the client signs off, do not let "one tiny thing" reopen it — that is what v4 is for. The lock is the protection.
